Feedback from three Age-Friendly Health Systems—NYC Health + Hospitals/Bellevue, Jamaica Hospital Medical Center, and Ryan Health—highlights that the tools are most effective when offered in both digital and physical formats. While digital access provides convenience, clinicians noted that many patients still rely on paper printouts due to limited access to reliable internet or smartphones.
Evidence from the program suggests that cost-related discussions remain a higher priority for family caregivers than for patients themselves, especially among those covered by Medicaid or Original Medicare. Caregivers often serve as the primary navigators for logistics and long-term financial planning, making the clarity provided by these decision aids essential. Ray Campbell, President of FAIR Health, noted that the success of these trials in New York is already guiding the organization's national expansion efforts, supported by a grant from The John A. Hartford Foundation.
