Commonwealth Care Alliance Appoints Dr. Pablo Hernandez as CMO

With over two decades of experience navigating the complexities of Medicaid managed care and community health, Dr. Pablo Hernandez has been named the new Vice President and Chief Medical Officer at Commonwealth Care Alliance, a move aimed at bolstering the organization’s integrated care model for Massachusetts’ most vulnerable patients.

Dr. Hernandez brings a career defined by his work with dually eligible individuals—those qualifying for both Medicare and Medicaid. Before joining CCA, he served as Chief Medical Officer for the Edward M. Kennedy Community Health Center in Worcester, where he managed clinical operations and population health. His background also includes a tenure as Senior Medical Director at Boston Medical Center HealthNet Plan, now known as WellSense Health Plan, where he directed the Senior Care Options product.

At CCA, he will oversee clinical leadership as the organization seeks to address the social drivers of health and expand its personalized care delivery. President John Koehn noted that Hernandez’s expertise in engaging government stakeholders and local providers will be critical as the company scales its high-quality, whole-person care initiatives. An alumnus of the Harvard School of Public Health and the Universidad Central de Venezuela, Dr. Hernandez maintains academic ties as a faculty member in Family Medicine and Community Health, while actively serving on state advisory boards and committees focused on delivery systems.
