Instead of the current regulatory path, the group proposes a mandatory registration and accreditation framework to ensure vendor oversight and clinical accountability.

Coalition Challenges CMS Remote Monitoring Rules with Registration Plan

The Save Remote Patient Monitoring Coalition suggests a Registered RPM Organization (RRO) framework to address concerns raised by the HHS Office of Inspector General regarding program integrity. By requiring vendors to enroll with CMS, undergo outlier-focused audits, and report annual outcomes, the coalition aims to provide the oversight regulators want without compromising the services rural and primary care practices rely on.

Third-party organizations are essential for many practices that lack the resources for continuous in-house monitoring. Proponents of the RRO model point to successful precedents, such as the accreditation standards used for durable medical equipment suppliers and the Medicare Diabetes Prevention Program. ChartSpan, which is spearheading the effort, highlights significant clinical gains from its own programs, including a 55.3 percent hypertension control rate and improved ejection fraction in 66 percent of monitored heart failure patients.

Jon-Michial Carter, co-founder of ChartSpan, argues that making these organizations visible and accountable is a more effective strategy than pricing a preventive benefit out of reach for vulnerable patients. The debate centers on the 2027 Physician Fee Schedule, with public comments due to CMS by September 14, 2026.
